• Internet. Fill out the form below and click the submit button.  After you submit the information, you will receive a confirmation email back.
• Email. Email FROI (First Report of Injury) medical records and appointment requests to wecare@iuhealth.org
• Fax. Fax FROI, medical records, and any orders to 317-216-2422, Attn: Customer Care.
• Phone. Call us at 317-216-2424 or toll free at 866-867-5555 to give us the information over the phone.
